Enter your information below to receive a prepaid FedEx or UPS shipping label via email.

Please package a minimum of 10 samples per shipper to avoid shipping fees. Include a completed Chain of Custody form with your samples.

Shipping labels will be provided during regular business hours (M-F, 9AM-5PM).

All shipments are for Priority Overnight service (Delivery by 10:30AM the next day)
